Project Participants

Quality Insights Renal Network 5 is hosting the 2018 Outcomes Congress on October 17, to congratulate all facilities participating in the Bloodstream Infection (BSI) and Vocational Rehabilitation Quality Improvement Activities (QIAs) on their hard work during this project cycle. Attendance is FREE but mandatory for project participants.

*Check-in is mandatory and begins at 9:00 a.m. Lunch will not be provided. There is a scheduled break from 11:30 a.m. - 12:30 p.m. during which attendees can have lunch offsite at one of Fredericksburg's many dining establishments.

Each facility is required to send one representative, preferably the person who led the project in your facility.  This representative will be responsible for producing a story board, sharing identified best practices, and contributing to the success of the event.  Register online.

StoryBoard Requirements

Each facility is required to develop a story board and deliver a 10-minute presentation highlighting the most influential intervention that was implemented in the facility.  This story board documents the progress the facility has made over the past year as it relates to the QIA.  Facilities should use their monthly reports as a blueprint.  Download additional requirements and a guide for story board layout.
